🚨Randomized data to support the use of LDRT for Osteoarthritis

In discussions with orthopedic surgeons about low-dose radiation therapy (LDRT) for osteoarthritis, the negative randomized data from Dutch studies are often cited as reasons to dismiss this treatment option. However, it’s crucial to recognize that these studies have significant limitations, including poor patient selection and small sample sizes, which compromise their validity and generalizability. More importantly, many colleagues are unaware of the existence of randomized data that strongly supports the use of LDRT for osteoarthritis, demonstrating significant improvements in pain relief and functional capacity. To make informed decisions about LDRT’s potential role in osteoarthritis treatment, it’s essential to consider the entire body of evidence, critically analyze study methodologies, and maintain a patient-centered approach rather than dismissing the therapy based on limited evidence.

❇️A recent Russian randomized study focused on knee osteoarthritis explored the impact of low dose radiation therapy (LDRT) with orthovoltage in diminishing disability risk among patients. The study compared the effectiveness of standard treatment with nonsteroidal anti-inflammatory drugs (NSAIDs) and symptomatic slow-acting drugs for osteoarthritis (SYSADOA) against the same treatment combined with LDRT.

Here are the key findings from the study:

📚 Study Design and Participants
The study involved 292 patients with confirmed knee osteoarthritis in an open randomized controlled trial. Participants were split into two groups: one receiving standard treatment and the other receiving standard treatment along with LDRT at a total dose of 4.5 Gy.

❇️Disability Risk Reduction
Results revealed that the group receiving LDRT experienced a notably lower risk of disability. Only 9.5% of patients in the LDRT group developed disabilities compared to 17.8% in the control group. The hazard ratio (HR) for disability in the LDRT group was 0.49, indicating a 51% risk reduction.

❇️Adjusted Outcomes
When adjusting for factors like sex, age, radiographic grade of osteoarthritis, pain intensity, and osteoarthritis duration before treatment, the risk reduction became even more significant (HR=0.24).

❇️Preventive Potential
Incorporating LDRT into the treatment plan could potentially prevent close to 50% of disability cases in knee osteoarthritis patients, with one in four disability cases potentially avoidable if LDRT was universally implemented.

Conclusion
The study underscores that combining LDRT with standard osteoarthritis treatments substantially lowers the risk of disability in knee osteoarthritis patients. This integrated approach proves to be more effective than standard treatment alone, offering a promising avenue for managing this condition. #radonc #orthopedics
