As a medical oncologist, this is one of the most disturbing breast cancer papers I have read recently.
Despite all advances in screening, awareness, and treatment, the incidence of de novo Stage IV breast cancer continues to rise. Even younger patients and men showed increasing rates!

Combining trained detection dogs indications with a Bayesian framework incorporating historical dog performance and participants features yielded 90.8% sensitivity & 91.3% specificity for multicancer detection from breath. Truly the human’s best friend! 🐶 https://t.co/YoSBWCGVWO
Our patients are vulnerable and some buy into this. It’s our job to continue discussions around misinformation like this!
He uses breast cancer as an example… so to build up on it, biopsy is essential for us to appreciate the TYPE of breast cancer so that we can recommend the right treatment (endocrine therapy, anti-her2 agents, or perhaps chemo-IO). Also, it can help adequately stage if we are worried about LN involvement so that we don’t UNDER or OVER TREAT!

TNBC is no longer a chemo disease.
This 1 algorithm shows the shift:
👉 ADCs + biomarkers are now driving frontline decisions.
🧬 Metastatic TNBC - how to think today
PD-L1 ≥10
• Pembro + chemo
• OR Pembro + SG
PD-L1 <10
• SG or Dato-DXd now competing with chemo
BRCA-mut
• PARP inhibitors
HER2-low
• T-DXd option
Later lines
• ADC sequencing
• Biomarker-directed therapy
🧠 Paradigm shift
❌ Before: one-size chemo
✅ Now: IO + ADCs + PARPi + precision oncology
⚠️ What matters next
Not just “which drug”
👉 but “what sequence”
Topo-1 ADCs → real cross-resistance concern
🎯 Takeaway
Advanced TNBC has entered the ADC era.
Smart sequencing will define outcomes. 🔖

The first rule of medicine is: first, do no harm!
In oncology, that does not mean feeling obliged to do something for every patient; it means knowing when to avoid unnecessary treatment. Not every chemotherapy regimen, every combination, or every later-line treatment provides real benefit. Sometimes the right decision is not to add more treatment, but to stop the toxicity. Knowing what to give matters; knowing what not to give matters even more.

Suicide is now the LEADING cause of death for US medical residents.
We are losing 500 physicians a year. That is an entire med school class. Stop demanding "resilience" from doctors and start fixing the system.
Tie executive bonuses to wellness, not just RVUs.

Who is a good oncologist?
A remarkable JCO article attempts to answer this question.
A good oncologist is not merely someone who delivers evidence-based therapy.
🔎 A good oncologist:
• Puts the patient at the center
• Builds trust and meaningful relationships
• Communicates with clarity and compassion
• Possesses emotional intelligence
• Manages the burden of cancer care
• Balances hope with realism
• Commits to lifelong learning and leadership
And my perspective:
A good oncologist is not someone who treats disease strictly according to guidelines,
but someone who never forgets that the person in front of them has emotions, fears, anxieties, and a family — a social life beyond the diagnosis.
Perhaps a good oncologist does not only treat cancer,
but also carries the human being facing it.

Being an oncologist is a great profession, but the emotional burden is heavy!
Oncology is the practice of maximizing benefit while balancing toxicity under severe time constraints. Add bureaucratic workload, and you end up ranked third on the list.

¿Que me rebajen mes a mes parte de mi salario para que a una persona en pobreza le atiendan el parto o le hagan una operación o le den tratamiento oncológico que de otra forma no podría pagar?
Sí. Siempre sí.
Most doctors treat medicine as l'art pour l'art.
But medicine is humanity science: It is much more important to know what sort of patient has a disease than what sort of disease a patient has.

What is the cause of ADC-related ILD? Spoiler: most likely not the antibody target (e.g. not HER2). New data confirms prior observations that T-DXd affects alveolar macrophages (AM) via Fc–FcγR engagement, triggering immune activation and ultimately ILD. https://t.co/mxtchICb8W
