Ever wonder why d-TGA or DORV occurs? It’s all about under-rotation of the outflow tract (OFT). As shown in our recent paper in Circulation, the aortic valve (red arrowhead) has to rotate to 6o’clock in the OFT where it meets the LVOT. If the OFT doesn’t rotate, the pulmonic valve (blue arrowhead) remains at 6 o’clock, leading to incorrect hook up with the LVOT (dTGA).
