FDA granted accelerated approval to adagrasib (Krazati, Mirati Therapeutics) for KRAS G12C-mutated locally advanced or metastatic non-small cell lung cancer after at least 1 prior systemic therapy. FDA also approved two companion diagnostics for Krazati.

Additional practice-informing adverse event patterns and management of #adagrasib (MRTX849) in patients with #KRAS #G12C-mutated #NSCLC.

GI-related TRAEs were the most common, but manageable. Occurred early and resolved quickly, resulting in a low (7%) discontinuation rate.

Not surprisingly, upstream feedback activation of wild-type RAS confers resistance to #KRAS #G12C inhibitors. This can be circumvented by convergent upstream (e.g. #EGFR) and/or downstream (e.g. #MEK) signal blockade.

KRYSTAL-1 just dropped on NEJM and the authors repeatedly highlighted the longer half-life of adagrasib. Then why is adagrasib dosed BID while sotorasib with a λ thats nearly x5 shorter is dosed daily?!

Actual ORR could be higher as 15.2% "not evaluable" cases were counted as no response. This study also suggested an unmet need for patients with #KEAP1 co-mutation.
