We generally use TEER when the anterior leaflet is elongated or when other mechanisms of MR are present. We also use it when septal reduction therapy is insufficient to eliminate SAM.

The HFH curves diverge early which may be explained by trial design- crossover to device arm was allowed after 30 days if a heart failure hospitalization where IV diuretic was administered occurred. This simple difference in trial design may be a key factor.
