We have few safety specialists in the NHS. People lack the training you see in other industries to deal with problems. Avoidance is the safer bet. Over-promoted managers lack expertise and aren't supported. Failing upwards and sidewards is a common complaint 7/9

You personally wedge #PAC at bedside and obtain a mean wedge 17 mmHg (a normal mean wedge is 6-10 mmHg). Admission wedge was 24
