@tdbauch Transducer should be leveled and zeroed to atmospheric pressure at the 4th intercostal space when measurements are obtained the sitting position.

But you can’t be wrong getting a VQ scan in someone with hx of PE/DVT who has SOB and concern for exercise induced PH. VQ is simple and Low risk and can take CTED and CTEPH out of the differential if negative

If TDCO was used, patient clearly have evidence of pulm vascular disease and I would get a VQ scan. Respiratory variation would affect the mPAP and PCWP equally and is not expected to increase the PVR in such a way
