Thresholds may guide care, but they cannot capture the context clinical judgment provides. Was a cutoff crossed — or missed — due to measurement uncertainty? How might that threshold shape access to care?

A good comparator indeed "Compared with what?" @NEJM https://t.co/cRDeITXqgG "As part of this process, investigators should be careful to compare outcomes associated with the use of AI with outcomes in the health care systems we have, rather than the ones we wish we had."
3 in 10 U.S. adults use AI chatbots for health information. Almost 1 in 5 say they cannot afford to see a provider.
@FDA asked how it should judge these systems. My answer starts w/the comparator.

We don't ask whether a model touched their prose. We ask authors to own every word.
@JACCJournals parted w/ICMJE & doesn't require disclosure of AI writing help: the rule is unenforceable, biased, & not good for medical communication.

@JXW43@JACCJournals It depends on the situation. The patient. The scope of the issue. For some, it is still like 1950. For others, it is like 2050. It is a transitional moment.
Search engines gave patients documents. LLMs give them an interpretation and a plan.
The information asymmetry that reinforced the physician's authority is eroding. Where that leaves physicians, in my Editor's Page (free to read): https://t.co/jp4vWtqvT0

My town will vote again, every spring.
The bill is always paid. Our responsibility is to make sure the care it buys is worth what patients and communities must give up.

This spring, my town voted three times before it could pass a budget.
Guilford, Connecticut. 22,000 people. The debate had the texture those debates always have — schools, roads, fields, what a town owes itself.
Almost no one named the largest force driving it.
The goal should not be less care.
It is less care that does not help, more reliable access to care that does, and greater protection from the financial harm of receiving it.
We should be as literate in a patient's financial exposure as in their ejection fraction.
New from the @ACCinTouch:2026 ACC Clinical Guidance on Adult Immunizations as Part of CV Care. Honored to serve as Vice Chair of this effort. Evidence growing that vaccines influence CV risk reduction not just infection prevention. In press @JACCJournals https://t.co/5emXtjAuie
