@dr_coops @bennyjohnson Agree there's no point in conjecture without good evidence....an insult to the professionalism and restraint the rest of us practice. It is unusual to find metastasic CaP when we presume he's been screened. Suggests aggressive disease in my mind. Very sad for him.

@uroegg @rewcastle_john Well said. Most obvious example is rebiopsy of GG1 is still recommended by NCCN...even in patients I may not biopsy in the first place due to pre- biopsy low risk. In those cases where biopsy is unlikely to disclose pattern 4, why do it? Screening=surveillance is apt here.

@EPotterMD I only do P2P with the patient present so that I can be reimbursed for my time and so the patient can see how ridiculous the process is. The most recent P2P the doc got on the phone and simply told me why they were denying and explained the appeal process. Patient was furious.
