I saw a case like this in an HIV positive patient who developed a photosensitive dermatitis then vitiligo in the areas. Interfacey on biopsy

Relapsing-remitting necrotic papules in an HIV+ patient=
Cryptococcosis (gelatinous reaction)!
-Little inflammation
-Mucoid tissue degeneration
-Small yeasts 4-20um
-PAS GMS mucicarmine +
-Sometime has granulomatous reaction

-Dense foamy macrophages superficially
-Elongate granulomas following the neurovascular bundles
-Positive Fite stain
Leprosy! (borderline)

@rqbittenc @JMGardnerMD @PIorfino @Dermpathl Thanks for the tag. Agree with seeing the overall architecture/symmetry at low and history. I suspect this will be benign. She’s young and these lesions can be very lentiginous and often have papillary dermal fibrosis. IHC for junctional density/pagetoid cells? Is it out?

Thought proliferative nodule but atypia too much for me. Malignant blue/melanoma in congenital nevus (banal nevus nearby?)/malignant neurocristic hamartoma (didn’t get a good feel for junction)

-Dilated follicle with parakeratotic plug
-Infundibular spongiosis
-Perifollicular lymphohistiocytic infiltrate (foamy histiocytes may be seen)
