Effect of Screening with Multicancer Early-Detection Test on Late-Stage Cancer Diagnosis

A large randomized English🇬🇧 trial involving 142,250 adults aged 50–77 evaluated annual multicancer early-detection blood testing alongside usual care.
➡️ After three screening rounds, there was no significant reduction in stage III/IV cancers compared with usual care alone (incidence rate ratio 1.03; P=.63).
➡️ Stage IV cancer incidence was lower with screening (IRR 0.86), although the result requires further follow-up. Trial-related adverse events were uncommon and nonserious.

The OlympiA trial shows that one year of adjuvant olaparib provides sustained benefits in patients with high-risk, HER2-negative early breast cancer and germline BRCA1/2 pathogenic variants.
➡️At 6.1 years, olaparib improved invasive disease-free, distant disease-free, and overall survival, with a 4.4% absolute six-year OS benefit.
➡️Benefits were consistent across subgroups. Toxicity remained acceptable, with no increased risk of MDS or AML, supporting continued adjuvant use.

The post-CDK4/6 inhibitor landscape in HR+/HER2– advanced breast cancer represents one of the most pressing unmet needs in oncology. While CDK4/6 inhibitors combined with endocrine therapy have firmly established themselves as the frontline standard, disease progression on this backbone remains nearly universal, driven by heterogeneous resistance mechanisms including ESR1 mutations, PI3K/AKT/mTOR pathway alterations, and RB1 loss.

This biological diversity highlights the limitations of one-size-fits-all approaches. Next-generation oral selective estrogen receptor degraders (SERDs)—such as elacestrant, camizestrant, and imlunestrant—are reshaping the field.

These agents achieve more complete estrogen receptor degradation than fulvestrant, show activity in ESR1-mutant disease, and are being actively evaluated in combination strategies with CDK4/6 and targeted therapies.

Together, they are helping shift the post-CDK4/6 setting toward a more dynamic, biomarker-informed treatment paradigm.

Premenopausal women with HR+/HER2-negativeearly #BreastCancer face distinct biological and clinical challenges, with important evidence gaps remaining.
➡️ Endocrine therapy💊 , ovarian function suppression, and CDK4/6 inhibitors require careful individualization, while genomic 🧬 assays may be difficult to interpret when considering chemotherapy.
➡️ Treatment decisions must also address toxicity, adherence, fertility and pregnancy, and survivorship. A shared decision-making approach is essential to balance recurrence risk, treatment burden, long-term health, and patient preferences.
