I have just seen a 39 year old woman who had presented with a cUTI; she showed me a Microgen report claiming her infection was cefalexin resistant. Anyway I treated her with cefalexin and she is back to a normal life after four years of misery. Treat the patient not tests.
Increasing water intake does not treat UTI. The microbes are dividing in the tissues; diluting the urine does nothing. The illusion: Given a positive culture, dilution of the urine leads to a negative urine culture. A trick using a test defect. You have not treated anything.

I feel so angry. What is a scientific conference doing hosting persons marketing complementary & alternative medicine? What should our patients think? Why a med. school scientific education? The doctors stand proxy for patients who then pay. Do you want this from your doctors?
The patients suffer terribly. They are desperate. How dare you profit from them without scientific justification? How dare you your slovenly data? How dare you disdain science? How dare your callous indifference to your customers? Anecdotes? Why do we tolerate such opportunism?
At the IUGA conference: Challenged guys promoting, to doctors, evidence-free, herbal extracts. Ill-informed; they were flogging dipsticks too. I was tough with the science: Unmoved. Exploiting patients with wrecked lives? Unmoved. Patients pay? Unmoved. Profiteering! Fight back!

So @NICEComms are willing to put time and effort into improving sample collection, but where is the work acknowledging the growing body of evidence that however 'clean' a sample you send to the lab, standard cultures are unreliable.

It's all very well getting a 'cleaner' sample, but the dipstick is unreliable, and culturing can only capture fast growing pathogens that grow on agar and may NOT be the pathogen causing the infection. No growth /= no infection. Women with rUTI know their bodies.
