Chimeric antigen receptor (CAR) T-cell therapy, which is an immunotherapy against B-cell antigen, has transformed treatments for cancers including leukemia and lymphoma. However, it can lead to severe neurological complications related to immune effector cell–associated neurotoxicity syndrome (ICANS) due to cytokine release leading to inflammatory changes. In this volume of Clinical Neurophysiology, in a retrospective multi-centered study, Freund et al. showed that older age and generalized slowing in pre-treatment EEG was associated with subsequent development of ICANS. These results suggest that pre-treatment EEG could play a role in the risk-benefit analysis of treatment decisions, including treatment dose and frequency of monitoring.
