Heart failure care still breaks down at the same point: congestion.
Despite guideline-directed medical therapies (GDMT), patients decompensate because we’re late in recognizing rising filling pressures.
Remote pulmonary artery (PA) pressure monitoring can help change that equation.
I increasingly think of it not as an adjunct or device strategy, but as a foundational pillar of HF management, alongside GDMT.
Why?
Because PA pressure data:
• Makes congestion visible before symptoms appear
• Supports proactive, precision medication titration
• Helps optimize GDMT in a safer physiologic state
• Enables real-time risk stratification and trajectory tracking
• Keeps us ahead of the disease, not reacting to it
GDMT modifies the disease.
PA pressure monitoring with tools like Abbott’s #CardioMEMS device can
inform timing, sequencing, and intensity of their care plan.
When used in the right patients at the right time, it becomes the connective tissue between therapies, turning episodic care into continuous care that is not dependent on the bricks-and-mortar clinic.
If congestion is the final common pathway in HF, then measuring it proactively should be core, not optional.
The future of HF care is not more rescue. It’s an earlier signal, smarter response, and sustained stability.

Here’s my simple logic on this. Perhaps too simple. Total VAD volumes are dropping in many places due to this challenge. If we look back at 2020 and learn that total VAD/HTX cases performed in the US are declining —that seems to mean we have failed to provide access to therapy.
