"Specialists for Pre-Hospital Endovascular Resuscitation & Extracorporeal Support" (SPHERES), resuscitationists advancing E-CPR for cardiac arrest in the field
5 of 5
SPHERES is an international collaboration (a "working group") for resuscitationists dedicated to prehospital endovascular resuscitation. The aim is to help build prehospital endovascular resuscitation programs worldwide, share experience, and advance science in this area.
About 10 years ago, a patient who's aorta I fixed donated £20,000 to us via @Barts_Charity. Since then this money has supported nurses in getting advanced degrees & training, junior doctors in education and presenting their own studies, pump primed research programmes, supported engagement activites and some of this work now has true national impact for patients. The donor always wanted to remain anonymous but we are eternally grateful. Even small amounts of money can go a long long way. 🙏
With the giants of Resuscitation Science who defined CPR compressions defibrillation, blood flow physiology, airway management and many other resuscitation aspects at the 17th Wolf Creek symposium ! Honored to have them all as my mentors over the years!!
Whole body CT scans after trauma detail the full injury load & improve coordination of on-going management.
Secondarily, they inform underlying health status (coronaries, CKD, sarcopenia) and are an unscheduled health screen.

Ultra-early management of pts with CA on #ECPR
🧠 think to brain: sedate! start pre #ECMO, not only for neuroprotection (pt memories?) + be cautious with FdO2 (I/R injury!)
☢️always go to cath lab before moving to #ICU unless evidence for PE (CT first)

How to manage patients with refractory cardiac arrest undergoing #ECPR after cannulation?? @JonMarinaro#ParisRescue
🖥️ TEE to support monitoring/weaning (details in slide ⬇️)
☢️ whole-Body CT scan may be advisable if feasible... check on @asaiojournal

🎈1st line to ⬆️ P vs epinephrine?
🎈between 1st/2nd line to extend window allowing to tranfer pt to #ECMO center?

How many lives did you save as result of your interventions? What is the chance of survival for pts with refractory CA after 15'/16'? Apporaching 0. If you get 20% survival rate with your #ECPR program, this is an impressive increase in chance of survival @ZackShinar#ParisRescue
#ParisRescue key messages
🔑 successful #ECPR starts w optimal bystander BLSD
🔑 integrate #ECLS assisted resuscitation in your system
🔑 #ECMO? multidisciplinary effort: for good outcome you need to involve intensivists, surgeons, perfusionists, nurses...

🗺️ how many cardiac arrest do you have in your area? how many are shockable? how many pts have positive neuro-outcome??
