- Data from 8 RCTs with 2705 pts (50% treated with Elo-containing triplets/quads).
- RRMM: Elo group had superior PFS (HR: 0.70; 95%CI: 0.60-0.82, P<0.001, I2=0%), particularly if PI/IMiD-exposed→ consistent benefit in high-risk RRMM (HR, 0.62; 95%CI: 0.43-0.90, P=0.01, I2=0%)→ ORR, ≥VGPR rate, and OS improvements also observed.
- NDMM: No significant difference in any efficacy outcomes.
- Safety: More G≥3 infections with Elo, but no differences in G≥3 cytopenias, G≥3 cardiac disorders, or SPM rates.
- As the only FDA-approved anti-SLAMF7 drug, Elo remains potentially valuable for overcoming resistance in heavily-pretreated RRMM→ should be reserved for later lines (not upfront!) with attention to infection prevention.

Their findings suggest that DOACs might be as effective as warfarin in preventing VTE without raising the risk of GI or intracranial bleeding complications. Without an RCT to answer this question soon, this paper provides new evidence on the role of DOACs in patients with MM.

KRd-treated pts showed higher odds of achieving MRD negativity and CR/sCR compared to VRd-treated pts, while ORR, PFS, and OS were similar between these groups. Among pts with high-risk cytogenetics, KRd induction was associated with a significant PFS benefit (HR=0.70; 95%CI=0.50-0.97; P=0.03; I2=0%). #mmsm
