The effect of tumor downsizing on surgical complexity during nephrectomy after immune checkpoint inhibitors for metastatic renal cell carcinoma

This multicenter retrospective study aimed to assess the oncological outcomes of delayed nephrectomy after immunotherapy in patients with metastatic renal cell carcinoma, specifically those who achieved a complete response (CR) or major partial response (mPR) at metastatic sites. Among 102 patients, radical nephrectomy was the most common procedure, with significant surgical challenges such as adhesions and inflammatory reactions occurring in 65.7% of cases. Tumor downsizing following immunotherapy was associated with increased surgical complexity, especially in partial nephrectomies. Despite these challenges, nearly half of the patients remained progression-free after a median follow-up of 29.6 months, with better recurrence-free survival observed in patients with CR at metastatic sites.
The study suggests that while delayed nephrectomy after immunotherapy can be complex, it yields favorable oncological outcomes. #KidneyCancer
