Important to be clear:
#SuicidePrevention is no less vital for mental health care & society more widely.
Having mental capacity is not a reason for services to say they cannot intervene with suicidal patients.

2.Myth: No need for HF in testing; errors are obvious.
Truth: No errors ≠ perfect design. HF uncovers hidden issues & user workarounds. Details: https://t.co/eI6RaEDx1Q
A successful health system must have the patient's voice and their needs at the heart of it.
The Patient Safety Commissioner has published a set of principles that all services should benchmark themselves against when considering how they meet the needs of patients.
The details of the principles and the College endorsement can be found here: https://t.co/KDXvd6vug6
Wishing all AHPs, support workers and allies a happy AHPs’ day. Thank you for all you do everyday. We have some incredible people @LPTnhs and @NHFTNHS so enjoy the day and celebrate your amazing professions. #ahpsday#weahps
The language we use as leaders shapes workplace culture. It plays a critical role in psychological safety. Our words (& the way we communicate) can encourage the experimentation & risk-taking we want our teams to demonstrate for innovation & improvement. It can also stifle initiative. @tom_geraghty sets out "seven deadly sins of psychological safety" - the most common/damaging things leaders say that crush psychological safety in the workplace:
1) “That’s a terrible idea.”
2) “You should know that by now.”
3) “Whose fault is this?”
4)“Everyone is replaceable.”
5)“Don’t bring me problems, bring me solutions.”
6)“Just get it done.”
7) “Not now, I’m too busy.”

This expert report from @MaryDixonWoods published today is an extraordinary summation of just about everything known on patient safety culture and systems, beautifully explained. A long but essential read for anyone working anywhere in the NHS https://t.co/zNIejZ8dFE
Culture is a patient safety issue.
Working environments effect quality and safety; they impact on staffing, on retention, and ways of working.
This World Patient Safety Day, speak up for safety.

Really sad to read of the loss of another NHS doctor to suicide. I had one of the best rotations of medical school in Stepping Hill A&E department in my 5th year in 2005. I was quite set on becoming an Emergency Medic. My experience there was so inspiring.
Happy International Nurses day. Looking forward to helping LPT nurses to use SEIPS /Human Factors methodology to create a system where we can all do our best work! 👏👏 https://t.co/XqDe07ZTap
🗨️I couldn’t quite understand how we were going to learn as an organisation, if we weren’t going to acknowledge the mistakes we had made in the past🗨️
