We are pleased to share our new work:
'Beyond the test: patient-reported concerns and worries regarding kidney conditions and #APOL1 genetic testing among Black patients'
🧬Among 200 Black adults in the #StLouis APOL1 cohort study with completed 3-month follow-up, 17.5% had an APOL1 high-risk genotype (2 risk variants).
🔵 At baseline, concerns about kidney conditions were substantial, with 39% of participants reporting being extremely concerned. At 3 months, concern remained unchanged in 45%, decreased in 36%, and increased in 19%.
🔵 APOL1 high-risk genotypes were more common among participants whose kidney concerns increased compared with those whose concerns decreased (29% vs 11%; P = 0.02).
🔵 Worry specifically related to having an APOL1 high-risk genotype declined from 14% at baseline to 8% at 3 months.
These findings highlight the importance of understanding the patient experience alongside genetic risk and of developing patient-centered education, communication, and counseling around APOL1 genetic testing.

In this piece, we explore practical opportunities to integrate AI into everyday nephrology workflows—supporting care before, during, and after the patient visit.
Our focus highlights how we can:
🔹 Improve Efficiency: Utilizing workflow AI to assemble information, draft documentation, and streamline routine clinical tasks to significantly reduce administrative burden.
🔹 Support Clinicians: Designing practical pathways that serve as a force multiplier for kidney care professionals.
🔹 Maintain Human Oversight: Ensuring that AI implementation remains responsible, with clinicians firmly at the center of verification and decision-making.
As the feature notes, our next priority as a field must be rigorous multicenter studies to evaluate the effectiveness, safety, workload impact, patient experience, and cost of these AI-enabled workflows.

It was an absolute pleasure to deliver my plenary, “AI in Transplantation — From Promise to Practice,” and discuss how artificial intelligence, machine learning, and advanced molecular diagnostics are actively reshaping the landscape of kidney transplantation.
During the session, we explored:
🔹 Complementary Diagnostic Modalities: Understanding what each test measures—how blood-based dd-cfDNA quantifies cell death and active graft injury, while tissue-based MMDx reads living graft cells to reveal active biological mechanisms.
🔹 Machine Learning in Biopsy Assessment: How MMDx couples whole-genome microarrays with machine learning reference sets to output objective probability scores for TCMR, ABMR, acute injury, and irreversible fibrosis alongside Banff classification.
🔹 The Multimodal Horizon: Fusing molecular diagnostics, longitudinal biomarkers, digital pathology, and clinical data to power AI-assisted surveillance, predictive decision support, and truly personalized post-transplant care.

🔹 Advanced Diagnostics: The integration of digital pathology, molecular diagnostics, and biomarkers
🔹 Decision Support: Leveraging multimodal data integration to empower clinicians and support precision medicine
Crucially, we will also discuss what is still needed to move from promising algorithms toward reliable, clinically useful systems that firmly preserve human oversight and ensure patient safety. I look forward to learning from colleagues across the transplant community and discussing how we can translate these advances into safer, more effective care.
