PAX5-altered adult B-ALL is not one disease.
A 42-patient cohort showed divergent outcomes by alteration; PAX5 fusions independently predicted inferior OS (HR 7.79).
Should PAX5 variant—not simply PAX5 status—inform risk?

Can low-dose ATG improve PTCy?
Among 97 PB allo-HCTs, ATG + attenuated PTCy vs PTCy alone lowered 2-y mod/severe cGVHD: 3.7% vs 17.0% (P=.046), without an observed rise in relapse/NRM.
Worth randomized testing?

Could antibiotics before day 0 weaken GVL?
Among 160 AML/ALL TBI allo-HCTs, early broad-spectrum antibiotics were associated with 2-y relapse: 45% vs 23% (sHR 2.00), without higher NRM.

Should CAR-T move before relapse in older B-ALL?
In 18 adults ≥55 in MRD− CR1, CD19 CAR-T yielded 18-mo EFS 84% and OS 100%, with no grade ≥2 CRS or ICANS.

Can an off-the-shelf bridge make CAR-T practical in T-ALL?
Triple base-edited CAR7 induced morphologic remission in all 11 pts; 9 (82%) reached deep remission and HCT. Seven remain in remission at 3–36 mo—but CD7 loss emerged.

Allo HCT remains a very effective though underutilized treatment modality for CLL. Cure rate ~70%, which remains higher than CAR-T. Having said that, I haven’t transplanted a CLL in a decade because I simply don’t get the referral.
We are entering a new frontier in AML ➡️The Paradigm study now out in @NEJM showing improved EFS in younger non-fav. risk AML Tx with Aza/Ven vs. Intensive chemo. Huge congrats to Amir Fathi & all authors for this incredibly important study & milestone. https://t.co/QiA8cHQKvc
In a randomized phase 2 trial involving adults with AML, azacitidine–venetoclax led to longer event-free survival than induction chemotherapy, with a lower risk of severe infection and hemorrhage. Full PARADIGM trial results: https://t.co/iiDNsw6CUW

Great work, congratulations to Dr. Fathi and colleagues! #AML#leusm
Summary:
The study met PE for EFS.
Higher ORR and CR in VEN+AZA gp
More pt received transplant in VEN+AZA gp
Better QOL
Huge congratulations to Amir Fathi for delivering one of the most impactful plenary presentations @ASH_hematology in AML in past decade. The PARADIGM study may lead to a Paradigm shift- Aza/Ven > intensive chemo in newly Dx high-risk AML. 👏👏👏
New opportunities for an “old” drug! The DEXAML-02 study raises the hypothesis that the addition of dexamethasone to standard intensive induction chemotherapy may improve the outcome of patients with acute myeloid leukemia.

SQ blinatumomab is highly effective in R-R ALL with CR/CRh rate of 77%, MRD-negativity rate of 91%, and 12-month OS rate of 63-70%.

1. CR/CRi rate 77% in RR B-cell ALL (compared to 42% with IV Blin), MRD 91% among responders.
2. Debulking required pre-Blin for BM blast > 50%.
3. Interval b/w cycles 1 weeks compared to 2 weeks with IV Blin.
4. GIII neurological adverse events were comparable.
5. Neurological AEs of any grade occurred at a higher frequency in the 500 μg/1000 μg group, the dose level of 250 μg/500 was RP2D #leusm
In allo-HSCT recipients, mixed chimerism, especially in #NKcells, predicts relapse. NK-cell MC stratifies relapse risk in MRD-negative patients. Routine NK-chimerism monitoring may improve post-transplant surveillance. #HSCT#MRD#Chimerism
