Let’s twist!
On-call ST elevation procedure started looking simple: 2 wire, 1 DES provisional strategy but…mind the gap!
Rupture of jailed wire so… 3 WHwire and twist!! That’s all!
@gmcioffi It’s not recommended remove side wire befor POT. If the side will close, you’ll lose a marker. The side wire could keep open the side. This was an unfortunately complication!
@jorgeachv Very open angle for diagonal, I prefer put a wire inside to avoid occlusion instead lose a lot of time to recross and give a lot of pain to patient
@kaucakv The jailed broken wire was not fully broken but stretch from diagonal to catheter. If u notice during maneuver the catheter went inside the LAD and after it went out quickly.
@OAB1967 In case of diagonal occlusion, it would have been very difficult to recross. One wire to avoid side occlusion, it was my choice to keep it simple

Let’s Rock! Nice case at Hospital “Barone Romeo” in Patti of LAD calcified stenosis treated with #rotashock approach (burr 1.5 -> POBA 2.0 -> shockwave 3.0 -> 2 DES 3.0-3.5) and good final result!
Most comprehensive meta-analysis (19,806 pts) with longest FU to date in EHJ shows coronary revasc in stable CAD reduces cardiac death (by 21%, P<0.001) and spontaneous MI (by 26%, P<0.001). Effect incr w/longer FU and similar in w/PCI vs CABG and w/w/o post-ACS or CTO. See poll.
