Could screening for clonal hematopoiesis help identify patients at higher risk of hematologic toxicity during radioligand therapy?

Clinical Impact and Dynamics of Clonal Hematopoiesis with 177Lu-PSMA-617 Therapy in Advanced Prostate Cancer

This study investigated clonal hematopoiesis (CH) in patients with metastatic castration-resistant #ProstateCancer receiving ^177Lu-PSMA-617 therapy. CH was detected in 66% of patients and was associated with a trend toward increased 📈hematologic toxicity, although overall survival was unaffected. Serial analyses revealed frequent emergence and expansion of DNA damage repair-related CH clones during treatment.

These findings highlight the need for further research into CH's clinical implications and its impact on radiopharmaceutical therapy.

We just published long-term f/u from OMNIVORE study in @jitcancer @sitcancer with @DrRanaMcKay, @MikeSerzanMD @BradMcG04 @BraunMDPhD @ZakhariaYousef @xiaoweimd, et al:

1) 50% of patients with #RCC can stay off-therapy with durable remissions after PD-1 inh. nivolumab induction
2) Adding CTLA-4 after PD-1 induction is a strategy that should be avoided if possible
