Many of my #AIinHealthcare posts are about future potential; this one is about current performance.
Hetairos, developed by researchers at @DKFZ German Cancer Research Center and the University of Heidelberg, predicts 102 molecular subtypes of CNS tumours directly from standard H&E-stained tissue slides.
These are glass microscope slides holding micro-thin slices of biological tissue that have been treated with two specific dyes: hematoxylin and eosin (yeah, I asked AI to explain this 😉), which is considered the global gold standard in 80% of medical tissue diagnostics.
This can change pathology, now! Instead of 12 days for a molecular workup, answers in as little as 12 minutes.
This is a 99.93% time compression.
It is effectively workflow obliteration!
Boom ... 💣
The model was trained and validated on over 11,000 slides from 9,606 patients across 11 facilities and four continents ... the data is robust and generalizable.
In high-confidence cases, accuracy reached 87%. And in a blinded comparison, it outperformed five board-certified neuropathologists on histology-only classification (68% vs 30%). It was tested in real diagnostic workflows, not just benchmarked in controlled conditions.
Here's why this really matters: DNA methylation profiling, as noted above is the gold standard, but it is expensive, time-consuming, and simply unavailable across most of the world.
On the other hand, Hetairos runs on hardware most pathology labs already have. This is a big #HealthEquity win.
Importantly, the five neuropathologists HetAIros outperformed were working without clinical context. The tool used age and tumour location alongside the slide. It wasn't a fair fight.
And to be clear, this is not a call to "replace pathologists". Hetairos narrows differentials and flags tissue regions of interest. It's a force multiplier for expert judgment, not a substitute and elevates the critical skills and judgment of a pathologist.
This is an example of AI as "infrastructure" done right in diagnostic medicine; prospective validation, real-world deployment, honest confidence scoring, and equity-conscious design.
