What is the value of HEMS?
Not just the intervention. Not just the aircraft. Not just speed. This study asks crews what HEMS actually adds at the scene.

Dear Med & Academic Twitter
Need your help in spreading the word about our latest OHCA study
This study aims to evaluate prehospital clinicians’ confidence in identifying underlying causes of OHCA, current treatment practices, team composition, and perceived barriers to improving patient outcomes.
We are looking for site champions from prehospital services across UK, Australia & New Zealand.
If you are interested please DM me

🔴SPEAR - DAY 2🔴
The complexity stepped up today with a deep dive into cardiac and cardiovascular physiology - focusing on what truly improves the chances of ROSC and positive neurological outcomes.
Morning scenarios pushed immersive simulation further, challenging teams to manage cardiac arrest through physiology-led decision making, monitoring, and targeted interventions.
In the afternoon, we made the most of the sunshine in our TAG outdoor space - exploring right heart & pulmonary physiology, plus aortic physiology, comparing full occlusion, partial occlusion, and adrenaline.
We wrapped up with four high-intensity scenarios: resus arrests, thrombolysis, REBOA, and prehospital cardiac arrest across varied aetiologies. The day ended with an al fresco EVTM REBOA masterclass from Halden 👏
The progression has been clear - teams are becoming sharper, more coordinated, and increasingly confident from fundamentals through to advanced endovascular techniques. Their CRM and leadership gets better by the hour…but as complexity increases, so does the challenge of managing and positioning kit and equipment in a logical way - there’s still plenty to learn, and tomorrow will bring more cardiac arrests and even more special circumstances.
Another incredible day of learning, great weather, and Danny’s elite BBQ skills 🍖
Day 3 - more arrests, more complexity, more learning - lets go!

In cardiac arrest, adrenaline reliably improves ROSC - but has not consistently translated into better long-term neurological survival.
Our comment asks whether the problem is not adrenaline itself, but how and where we deliver it..

1/ New from @EAAARAID in @SJTREM: observational review of Penthrox for prehospital fracture/dislocation manipulation & reduction in HEMS practice.
In selected limb trauma, methoxyflurane supported successful M&R without IV procedural sedation in more than half of patients.
Whole blood in trauma has generated enormous interest. Today, @SWiFT_trial in @NEJM provides the clearest randomised evidence so far on whether prehospital whole blood improves outcomes in life-threatening traumatic haemorrhage: https://t.co/iqgPNZNdfk
🔵ATACC SPEAR🔵
⚠️ Limited Spaces Remaining | 20–22 April 2026 ⚠️
