So as a junior myeloma faculty, I see a lot of consults for MGUS. Heres a tweetorial on how to interpret light chain values in the setting of kidney disease, as light chains are often checked to look for plasma cell disorders when patients have CKD.

My advice for expecting parents: Reject the cynicism.
There's a weird trend of cynicism around becoming a parent:
• "Have fun never sleeping!"
• "Hope you got your fun in!"
It's dumb.
The good outweighs the bad by 1000x. The frictions are a tiny tax on the joy it brings.
Sensible optimism: I’m going to do my best to put myself in a situation where the odds of success outweigh those of coming up short. There will be setbacks along the way, but with the right mentality I can power through them 💪🏽
Thinking of all #matchday2023 candidates today!
1/ There is more to serum protein electrophoresis (SPEP) than meets the eye. A SPEP is a rich exam, with a wealth of information, that has been relegated to the land of malignant hematology with advances in lab testing. Let's rediscover its richness through a few clinical cases.
We miss out on opportunities when we only ask what could go wrong. It's also worth asking what could go right.
Change carries risk: we might fail. But sticking to the status quo also brings risk: we might fail to grow.
It's better to test and learn than to never test at all.
Happy to announce our next endeavor!

Benjamin Franklin created a list of virtues that he believed would forge a life of health, wealth, and happiness.
200+ years later, the list still holds incredible wisdom...

1. Chronic metformin use can cause B12 malabsorption. Possible hypothesis- it interferes with Ca+2-dependent membrane action responsible for B12 IF absorption in terminal ileum
2. Serum ferritin level > 100 rules out Iron deficiency even in pts with inflammation
Today is my 32nd birthday.
Over the last month, I asked several 90-year-olds what advice they would give to their 32-year-old selves.
Here's the life advice everyone needs to hear:
