Excited for another busy #ATS2025, with @DrPeter_George and Prof Anand Devaraj presenting the latest evidence for our FDA-cleared e-Lung technology, which has been robustly validated in the #INBUILD study to identify progressive pulmonary fibrosis accurately and sensitively.
🔥🔥Patients with #ILD due to #SSc or #RA Need Monitoring More Frequently Than Annually
🔍 Methods
▪️Design: Post-hoc analysis of placebo arms in RCTs (SENSCIS and INBUILD).
▪️Population:
#SENSCIS: 288 patients with fibrosing SSc-ILD.
#INBUILD: 47 patients with progressive fibrosing RA-ILD.
▪️Outcome Definition:
ILD progression defined as ≥5% decline in FVC % predicted or death.
▪️Analysis:
Kaplan-Meier estimates for progression over 52 weeks.
Worst observation carried forward for missing data imputation.
✅ Results
🔹SSc-ILD: Progression rates increased incrementally, reaching 36.5% at week 52.
🔹RA-ILD: More rapid progression with 55.3% at week 52.
🔹Kaplan-Meier Curves: Demonstrated time to progression varied significantly between the groups, emphasizing a faster decline in RA-ILD.
💪Strengths:
🔹Clear Question: Addresses a clinically relevant gap in ILD monitoring practices.
🔹Robust Data Source: Utilized RCT placebo arms, minimizing intervention-related bias.
🔹Objective Outcomes: Reliance on standardized decline in FVC % predicted.

🔹Small Sample (RA-ILD): The INBUILD subgroup (n=47) risks underpowered conclusions.
🔹Post-Hoc Nature: Retrospective analysis limits causal interpretation.
🔹Data Imputation: Worst observation carried forward may overestimate progression.
🔹No External Validation: Findings are restricted to the trial population and lack real-world applicability.
💡 Conclusions
The findings challenge the appropriateness of annual ILD monitoring in autoimmune diseases, suggesting shorter intervals to detect rapid progression, particularly in RA-ILD.
🚩 Implications for Practice
Patients with RA-ILD and SSc-ILD should undergo earlier and more frequent assessments, especially within the first 1-2 years post-diagnosis.
Future research should validate these findings in real-world cohorts and include mortality-specific analyses.
