Christmas 2016: I’d just had my 1st great results on #PAPMET but still scared it’d be my last. I was skinny bc I couldn’t keep anything down, hair & brows had gone white, had blisters on my feet & tongue & was tired as hell on Cabo 60. Merry Xmas 2023, y’all. Miracles happen 🧡🎄

Thanks @DebMaskens for bringing this up. As you know, I invested considerable effort in #PAPMET. Despite the PFS advantage we saw w #cabozantinib v #sunitinib, guidelines (such as those from @Uroweb below; downloaded today!) give it a “weak” rating - this has a MAJOR impact on access. I’ve had certain insurance companies deny #cabozantinib for my patients in this setting. Can I fault guidelines (& resultant payor decisions)? Not entirely. As proud as I am of the study, I have to concede that #PAPMET was a RP2 study (only 152 pts across 4 arms - #caobzantinib, #savolitinib, #crizotinib & #sunitinib) & no significant OS benefit. Many have brought this shortcoming of the study up with me & I cannot discount their perspective. Also, in most of the world, #sunitinib remains the only available option. In trials like #SAMETA & #STELLAR304, the choice of control arm reflects what most of the world presently has access to in this treatment setting. I think that’s appropriate for a phase 3 #international trial. Of course, keep in mind that the eligibility criteria for studies in this space vary widely. #STELLAR304 includes #translocation & #unclassified #kidneycancer, for which there is an even greater paucity of evidence (and certainly no de facto standard). My hope is that the more sizable randomized P3 #STELLAR304 will offer more definitive evidence of what we should use in this setting. If positive, I think this study will finally change guidelines.
