Well stated. Academic-industry models in research require collaboration but also carefully defined independent systems of oversight through #DSMBs @califf001 @NMHheartdoc @CMichaelGibson @Drroxmehran @TheMahaf

Hopefully people understand this is why it’s important to do the randomized clinical trials with a system that oversees the health of participants

#DSMBs have an instruction set (charter) agreed on before the #trial starts, includes prespecified statistical analyses (very low p-values) and thresholds that would trigger stopping a trial early for efficacy (big beneficial effect), toxicity (harms), futility (wasting our time)

Not enough written about how #DSMBs do their work. Combines clinical and stats insight w experience. Perfectly reasonable to continue trial if later mortality finding might emerge.

The COAPT DSMB (Dave Williams, Spencer King and I) knew the heart failure primary endpoint would be positive for a long time. Mortality curves split later. Should have stopped trial early for efficacy? ⁦@HeartBobH⁩ ⁦@GreggWStone⁩ ⁦@SVRaoMD⁩
