@BichileT I would ask @TYoungstein as she’s my go to expert & the queen of #MAS. Fundamentally need to treat the #lupus so would give pulse #methylpred, oral pred, Cyclophosphamide & Rituximab But might use anakinra early on to reduce inflammation. Have used brief course CNI before

But in their day game changing. & if patients non adherent /intolerant with oral meds the #CyP #MethylPred plus #Rituximab or #belimumab can be life saving. #lupus #nephritis 4/

@hormone_doc @chriswincup My mantra! Almost every study that has used lower doses of #steroids, efficacy at least as good if not better and toxicity reduced. Max ##methylpred dose should be 500mg or less. #lupus #nephritis

@chriswincup One of my all time favourite references! 100mg #methylpred x 3 = efficacy 1000mg x 3 doses. Lower doses AS effective. Demonstrated in 1987!! #steroids #lupus #lessismore

Majority of 100+ #rituxilup #patients remain off steroids after years @hannahksb @doc_wilson3 @1mariecondon 1/

These studies differ in steroid (#hydrocortisone, #dexamethasone, #methylpred) & dose, population enrolled (ICU vs wards, intubated vs not), & primary endpoints (mortality, MV duration, organ failure, etc) All were well done methodologically though several were stopped early. 5/

@tulunsokit @GlassockJ @KavGul6 And agree with @NephRodby it’s a very nasty #biopsy with necrosis and I imagine losing function quickly. We’d almost certainly use #modified #eurolupus - much <#steroid (2-3 x 500mg #methylpred, 0.5mg/kg/d oral pred) & likely #Rituximab. And #ovarian #protection & #HCQ 2/

@Dr_Beale_ @arvindcanchi @akshayakeerti @dr_nbbbb Don’t disagree - my first line for #lupus #nephritis would be #rituximab #methylpred (500mg x/) and #MMF and NO oral steroid but expensive. Am assuming patient in India & may be unaffordable.

Relevant to so many areas of #nephrology #membranous #lupus #Vasculitis #IgA etc. #methylpred pulses should prob 500mg IV or less (can cite you why) in non #CNS disease, oral pred max 0.5mg/kg/d, max 60mg. We need trials - dose, duration, taper etc. #ERAEDTA18 @AuriniaPharma
