🔥 ILD Key Insights (JAMA Review)

• HRCT pattern is decisive → UIP strongly indicates IPF; non-UIP → CTD-ILD / HP / Drug-ILD.
• IPF is rapidly progressive → median survival 3–5 yrs → early antifibrotics essential.
• PF-ILD definition: ≥10% FVC drop OR radiologic/clinical progression despite treatment → treat as fibrosis.
• Nintedanib slows FVC decline across multiple ILDs; pirfenidone mainly for IPF.
• Inflammatory ILDs respond to steroids + MMF/AZA (not ideal for fibrotic ILDs).
• Autoimmune ILDs: SSc-ILD has highest fibrosis burden; RA-ILD has high mortality.
• HP improves with exposure removal + early immunosuppression (if inflammatory).
• Drug-induced ILD often improves after stopping culprit agent.
• Biopsy only when HRCT unclear → cryobiopsy increasingly used.
• Early ILD specialist referral prevents irreversible fibrosis.

Pattern first. Progression next. Treat fibrosis early.
