Electrical alternans is a rare condition characterized by alternating QRS amplitudes seen in one or multiple ECG leads.

It is secondary to the motion and rotation of the heart, causing beat-to-beat oscillation of the QRS wave amplitude of the ECG.

It is mainly associated with large pericardial effusion and "swinging heart" on TTE imaging but could also be associated to other cardiac conditions including arrhythmias, conduction pathway, abnormalities, and extracardiac pathologies like left-sided tension pneumothorax.

Although often associated with tamponade, it should not be considered diagnostic of hemodynamic compromise.
