JUST IN: LITESPARK-012 did not meet its dual primary endpoints of PFS and OS versus pembrolizumab + lenvatinib in frontline advanced ccRCC. A disappointing result, but an important one. Carefully conducted negative trials still move the field forward by challenging assumptions, informing future strategy, and helping us avoid adding complexity without clear patient benefit.

There are currently 4 global randomized III trials exploring novel ADCs in UC. 2 of them explore different TROP2/TOP1 ADC (like SG which failed for OS in UC) 👉SAC-TMT alone 3rd line 👉D-DXD in combo with platinum 2nd line. IzaBren looks more novel (HER1+3) while the DV/IO study has already been done successfully in China. How many of these will make a difference. @OncoAlert @Annals_Oncology

A 240 pateint single arm trial exploring 9 cycles of EVP without planned surgery in MIBC. This will answer the key questions ‘What happens if we don’t do cystectomy in those with clinical complete response after initial EVP’.It assesses cCR rates and bladder intact EFS. Nectin-4 is expressed in almost all urothelial cancers . Nectin-4 fluorescence imaging during cystoscopy makes sense and seems to work well (see below) #EAU26 @urotoday . This potentially improves diagnostic accuracy. It would be good to generate large randomized data. @OncoAlert

Discussion #EAU26 about the future of cystectomy neoadjuvant treatment and radiotherapy in muscle invasive bladder cancer. Things are changing rapidly. With pCR rates >50% for EVP many patients will want to keep their bladders. An era of ‘EVP 1st and ask questions later’ maybe a reality soon. Generation of EVP bladder sparing data is a priority. @Uroweb @EUplatinum

The big renal cancer story ASCO GU #GU26 are the 2 positive R3 belzutifan combination trials. Many feel belzutifan is well placed earlier in RCC which is more angiogenic & in combination due to a good tox profile. LS22 @DrChoueiri show adjuvant bel + pembro is better than pembro while LS11 tests adding belzutifan to lenvatinib vs cabozantinib in pretreated disease @motzermd @OncoAlert

HT + post-op RT did not significantly improve OS overall
HR 0.87 (p=0.06).

No meaningful benefit at PSA ≤0.5
potential LT-HT signal at higher PSA.

Pre-PORT PSA may help refine patient selection.
