It was always problematic when you were just phoning an unknown member of a large MH team or when they were trying to offload the patient to a different team. The Trafford/Manchester border was interpreted very differently by different MH team members.
Really wise words from @ProfLAppleby, especially the need for mental health teams to listen more to families - and I would add those GPs who still maintain continuity, so know their patients well. We had good success when there was a good relationship between GP and MH team but
I gave evidence to the Nottingham Inquiry on Friday.
In my view the Inquiry will shape mental health care for the next decade, as did the inquiry into the killing of Jonathan Zito in 1992.
I made 3 points:

Considerations for bringing back this exact same Bill (and it would have to be the same, if Parliament Acts is used):
1. The Royal College of Physicians have said her Bill is unsafe
2. The Royal College of Psychiatrists have said the Bill is unworkable, and unsafe
3. The British Geriatrics Society have said the Bill’s safeguards are not adequate
4. Domestic abuse charities have said the Bill is unsafe 5. Organisations representing disabled people have said the Bill is unsafe
6. Royal College of GPs says the Bill lacks adequate safeguards
7. Lord Stevens, ex NHS CEO, said legislating for assisted dying in the current climate of hospice cuts is “utterly ridiculous”
8. MIND says the safeguards are not adequate
9. The CLADD group at KCL have said the Bill is “not fit for purpose”
10. The British Association of Social Workers say the Bill’s is not safe enough
11. The Association for Palliative Medicine have said this Bill is unsafe
How about introducing a Private Members Bill to provide equitable access to palliative care across England and Wales? Rather than starting with a Private Members Bill for Assisted Dying? The sequence would be hugely important, given that so many cannot access basic care.
We have a lot of historical records of exam papers like this. Frustratingly, we have far fewer records of what the students actually wrote in response! Maybe their responses were all brilliant. Maybe some of them were terrible.
My colleagues wrote a paper on standards in A-level maths over time, and finding archive responses was really hard!
Eventually they were able to compare responses from 1964, 1968, 1996 and 2012.
Standards had declined dramatically between 1968 & 1996. The quality of work that would have got an E in 1968 got a B in 1996.

Look at this figure - a huge rise in non GPs in the workforce in England - but what has this achieved- greater range of services in general practice BUT higher FTE of non GP direct patient care staff is associated with higher A&E attendances - policy makers take note!! #GP
Interesting to hear @SKinnock make the point in the health select committee that the #assisteddying bill has been a positive thing because it has encouraged people to talk more about death and dying.
My experience is that it has increased fear about death and dying as the majority of the campaign has been about death being awful and something that must be controlled through ending lives early. Can he really claim this as a positive outcome? Particularly without robust evidence to support this?
We need to talk about #normaldying and not pretend a campaign to legalise assisted dying has provided the education the public need.

The CQC has for a long time failed to demonstrate that they understand the principles of well-led, effective and fair assessment. Their assessments are therefore unsafe. And they are anything but responsive.
Is anyone else taking part in the @CareQualityComm engagement webinar today? This was my question to @BolaOwolabi8. The chair advised that specific concerns should be raised via the CQC's contact centre. But we made a formal complaint on 22nd Oct and haven't had a response yet...
