If transporting adult resp failure pt on HFNP instead of PHI, need to calculate amount of O2 carried on plane (you might run out!) and consider risk of in-flight deterioration. HACOR and ROX scores might help predict risk of HFNP failure.
In this case report, we grapple with the eternal #PHRM debate of #stayandplay vs #scoopandrun in a patient with severe respiratory failure in one of the remotest regions in the world.
We already know prehospital intubation prolongs scene time thanks to @PFokEM@appearce2 et al. Plus there’s the risk of cardiovascular collapse in austere setting.
Aeromedical retrieval services like @rfds_co_egmmrs transport pregnant ppl in labour from remote Aus to birthing centres. How do we predict how quickly labour will progress and avoid birth on a plane?
… and here’s another one! Excellent paper on remote obstetric aeromed retrieval by @pekkad & @GardinerFergus with @likeadoc@RedOzRetrieval et al.

Many ppl did not birth during admission after retrieval. Unnecessary retrievals are disruptive for the person and family, culturally harmful and could indicate lack of services in very remote regions. Could better assessment tools like fFN help?
Fantastic interview @mabernoth! Insights into opportunities and challenges for registered nurses in aged care esp rural, and family perspective on accessing aged care services. Highly recommended listening for my #GeriEM colleagues
