don’t use linelozid in ESKD unless you HAVE to (daptomycin often a possible alternative). If you do, monitor CBC very closely. Even though off label, you may need to consider an appropriate renal dose.
To me this speaks to a larger problem of general lack of focus on nephrology in internal medicine residency. Nephrology simply cannot be an elective. It is a core area of internal medicine that requires good basic training to avoid these clinical scenarios.
Pt admitted for AKI. A/P by primary team:
"Baseline Cr < 1, Cr up to 2
Holding losartan & lasix
Giving LR 150 mL/hr continuous"
No comment on volume status & no mention of working diagnosis😲
Discharged on day 3 with "stable" Cr in mid-2s (4x baseline)
