Extended End-to-End Anastomosis via Left Thoracotomy #2: End-to-End Anastomosis

👉After clamping, completely resect the coarctation and all residual ductal tissue from both aortic ends.
👉Extend the proximal incision along the arch undersurface, with a longitudinal counterincision in the descending aorta.
👉Approximate the mobilized aorta and construct a long oblique running anastomosis to restore arch caliber without tension.

Perimembranous VSD Patch Closure 🎥

👉Surgical exposure is everything: clear definition of the VSD margins, aortic valve, septal leaflet of the tricuspid valve, papillary muscle, and chordae is essential.
👉The suture line must be planned deliberately to protect the conduction tissue, avoid aortic valve injury, and achieve a secure patch closure.

Mitral Valve Anatomy #1: Leaflets and Annulus

👉The mitral valve consists of the annulus, two leaflets, and two commissures functioning as one integrated unit.
👉The posterior leaflet has P1–P3 scallops, while the anterior leaflet is described by the corresponding A1–A3 segments.
👉Anteriorly, the annulus is continuous with the aortic valve through the aorto-mitral curtain, and forms a dynamic saddle shape.
