@VanessaWrites @kidneydoc101 The transition to work is tough, and many women feel the stress of balancing work/home. Having the flexibility to make it to pediatrician appointments and making breastfeeding at work easier were important to me. I had a private office and a fridge. Also water and snacks :).

@LVRiella I was under the impression that Jynneos is a live vaccine. Do you know of any data for safety in transplant patients?

@M_Elraggal Does he meet the criteria for lung cancer screening? Is this polycythemia chronic? Maxed on ACEi/ARB?
Personally I think risks are too high at the moment (in stent thrombosis and stroke). I would consider offering phlebotomy first and then offer SGLT2i.

@askrenal @NephJC @izharnephrol No, but usually I prefer anemia is optimized prior to biopsy, or any elective surgical procedure for that matter.

@askrenal @NephJC @zzvalika I would proceed with induction after careful discussion with the patient, especially if you believe there's a good possibility of renal recovery. Unfortunately so many of these conversations always feel like being stuck between a rock and a hard place.

@SamiAlasfar Somewhere in the back of my mind I remember being told that calcium channel blockers are better for diastolic hypertension, but will have to look for literature to support it now with all these other tweets.

@BrouwerMaier @HomeHemodialys1 I thought of this today after seeing a patient with a fully functional fistula and a permcath that he's been using for three years. He works lifting heavy items every day for work and hasn't been using the fistula for that reason.

@brian_rifkin @ghobby @hswapnil In transplant it's been helpful on occasion with providing a diagnosis, especially in younger patients who were late to present. Last week I had a patient who had a COL4A3 mutation c/f Alport's and will now been checking anti GBM.

@ghobby We have recently started using it for some of our transplant patients. I think it's been a good tool so far and has helped us find a few surprise diagnoses. I believe the company can connect patients with a genetic counselor.

@HomeHemodialys1 I'm a recent grad, and I can say that the nephrologists were definitely the best internists where I trained. They inspired me to go into nephrology. Having just finished some of our fellow interviews I think that sentiment still exists.
