Ukraine is paying a high price in blood and terror. They are not the first and will not be the last if Russia isn’t defeated now. This is the lesson I wish the West would finally figure out. The price always goes up. There will be no better chance than now.
📣 Just released! Joint @EANM_NucMed@SNM_MI Lu-177 PSMA guidelines. Step forward in improving the standard of care for delivery of radioligand therapy. @ProfKHerrmann

Metastasis-directed therapy (MDT) in prostate cancer with SABR/SBRT and sLND:
- postpone need for ADT
- improved PFS
- one study with improved OS with SOC + SABR

Terbium-161 may become an interesting alternative to Lutetium-177, here an example in PSMA-targeted Radioligand Therapy…. #Terbium#Tb161#RLT#PSMA#prostatecancer
It's a paradigm changing concept, uncomfortable to many: PET/CT images "live" pathology, and may be better than dead tissue biopsies in *some* circumstances.
#ProPSMA 54 month follow-up #EAU23 "live" @declangmurphy
✓ PSMA PET/CT N1 prognostic for failure - HR 2.1
✗ CT/BS not prognostic
= PSMA PET/CT findings significant. More and more longer term data supporting it's use.

Saw another patient this week misclassified as progression on DOTATATE PET/CT when the change was likely due to commencement of long-acting octreotide therapy. "Pseudoprogression" under-recognised phenomenon:

@CalaisJeremie@baumrp Sharply observed Jeremie! Not many dealing a lot with PSMA imaging have understood the huge potentialem late imaging! For radiation oncologists it is a great opportunity to apply salvage Rx in a more targeted or boosted approach with positive findings..
