Let me push this a little further, as this part is often left out of most academic conversations.

If we’re going to wring our hands over doctors leaving the system, I think we have to ask something uncomfortable:

Where are the hospital administrators in this sacrifice economy?

If the concern is that concierge and DPC physicians are “taking their ball and going home,”
then let me ask:

•Are health system CEOs willing to take less compensation?
•Will nonprofits give up their 340B windfalls, GME payments, or tax-exempt real estate empires in exchange for expanding access to care?

Because it seems to me, we’re only ever asking the people at the bottom of the totem pole to bear the moral burden.

Why is the answer always:

“Get back in the system. Be a good soldier. Take the burnout and the pay cut — for the greater good.”

But we never ask the $4 million-a-year (that's $320,000 a month) hospital exec to cancel their golf trip, cut their bonuses, or give that medical tower back to the tax rolls.

So I’ll ask it plain:
Why is physician autonomy the thing we regulate… and not hospital consolidation, academic subsidies, or corporate capture of Medicaid money?

We say we want to save the commons, but we’ve designed it so that only the doctors bleed to keep it alive.

Today, a physician working at the same productivity level is stuck with $32.36 per RVU—which means, adjusted for inflation, they’re effectively making HALF of what they used to.

Think about that: same work, half the pay.

Meanwhile:
Hospitals get sweetheart site-of-service fees.

Private equity buys up independent practices at fire-sale prices.

keeps slashing physician reimbursement.

Bureaucrats rake in raises while doctors get the shaft.

In 1992, the Medicare conversion factor was $31.00. Today? $32.36.

That’s a $1.36 total increase in 33 YEARS. If it had just kept up with inflation, it would be over $64.00 today.

Instead, it’s more than 50% below where it should be.

A doc today has to see twice the patients, do twice the procedures, and grind twice as hard just to keep up.

And what does Congress do?

No doc fix in the latest CR.
No relief.
No respect.

Just another reminder that the system values hospitals and middlemen over the people actually delivering care.

Physicians aren’t just underpaid.
They’re being systematically devalued.
