@DFCapodanno @drjohnm @NEJM the patients that do best with laao barely have afib and probably didn't even need oac. yet they all get included in trials as afib patients. if you want real answers and events, make a trial of elderly permanent afib patients. randomize to laao vs doac vs under dosed doac.

@eyadalhaj I had a young lady referred to me like this for icd implant. 12 lead ecg with inverted p waves. Stuck in slow AVRT. Recovered with ablation completely!

@rbarbosa91 I usually place 4 central femoral lines for my cases (ep) . After a med or np student has been with me a week learning us guidance and micropuncture technique I let them do them with me. They do fine. I learned intubation and radial arterial lines as a med student. No issues

@DaneshModi @BiosenseWebster @aalahmadmd I've had exact same experience, more to do with NGEN then qdot. Try to keep impedence more than 100 if not will need to turn down power to reduce current delivery. I've turned on current delivery on NGEN display, it does not show on Carto main screen

@ncurzen @drjohnm What changes would you make to AFIRE trial to repeat? Exclude cabg? A trial shutting down by IRB due to harm and no benefit of antiplatelet makes it really hard to recruit for a future study. I always get referred these pts for Watchman and I try stopping the antiplatelet first

@cellisvandyep Comparing patients undergoing LAAO to historical stroke rates 20 plus years ago isn't fair. With modern monitors/ILRs Patients with 1 day of afib in years are undergoing LAAO. These lower? risk patients would have never made it into the original CHADS datasets
