ISR in the LAD: when OCT tells the real story.
A patient presented with recurrent ISR in the LAD. OCT revealed clear features of in-stent neoatherosclerosis, with lipid-rich neointima and advanced neointimal transformation rather than simple neointimal hyperplasia.
This case highlights why angiography alone is often insufficient in ISR. Understanding the underlying mechanism is critical for selecting the optimal treatment strategy. Neoatherosclerosis is increasingly recognized as a major cause of late stent failure, recurrent ISR, and adverse clinical outcomes.
