Incorrect 'CPR' Sucks!
Early CARDIAC ARREST management only makes sense if you understand pathophysiology and you target interventions to the patient
Here I describe what's WRONG with many cases of cardiac arrest resuscitation and how we can fix it

BRUEs (Brief Resolved Unexplained Events) in infants can be really concerning for family members to witness. But, inherently, when prehospital teams arrive the infant is often alert and crying. So what's going on?
Here's what we need to know....🧵

JC secondary analysis of DOSE VF trial shows (DSED) a superior strategy over AP/VC defibrillation for refractory ventricular fibrillation (RVF) in OOCH, improving survival rates and neurological outcomes significantly. @stemlyns#FOAMed

Ok. A few bits. Will add a context of intravenous sedation for long bone fracture manipulation as an example. 1. Even when the pre-sedation environment is tense, noisy, loud, etc., speak gently with the patient before you start. Allocate someone to continue to speak smoothly and
Interesting study shared by @AndyPattonIRL at @SydneyHEMS Governance Day - TL;DR we underestimate large volume blood loss and overestimate small volume blood loss on scene (1/3)

Thanks to @cliffreid for spotting this one.

This is also our experience @SydneyHEMS - we have a large video library of severely contaminated airways from both prehospital and interhospital missions showing effective intubation with standard geometry VL using the Storz C-MAC device (no conflict of interest)
@DrBSingleton@cliffreid Well it can…. & this is an oft quotes limitation to VL…….but the available evidence suggests VL outperforms DL in the soiled airway.
This from a John Sakles ED airway registry
Of course happy to see other better evidence.
Please visit @stemlyns to read about Max and the reasons why we need the AMAX4 algorithm in anaphylaxis/asthma. I promise you'll learn from this tragic story. Thank you Ben and Tamara.
