Learning point from EM conference: In patients with syncope then an ED EKG with a conduction abnormality (Brugada, HOCM, ARVD), we must assume the rhythm at time of syncope was vtach. Vtach without a pulse is cardiac arrest. This is a post-ROSC patient! Treat accordingly!
