One of my missions is to make Peripheral Neuropathies more understandable for non-experts. Recognizing that physicians often find PNs challenging, especially in the hospital setting, I collaborated with @MayoClinicNeuro Neuromuscular hospitalists, @ReeceHass and @SantilliAshley, and my former NM co-fellow #JMartinezThompson to develop an engaging infographic that simplifies how to approach neuropathies leading to hospitalization. The incredible artistic skills and creativity of @ReeceHass truly brought this project to life, exceeding all expectations. We hope you find this helpful. https://t.co/HjEJbfOkqh

You can chase the PR and still love a Tuesday jog.
You can take the work seriously and stop thinking about it at dinner.
You can be ambitious and take the whole weekend off.
Wanting more and enjoying what you've got aren't opposites. We just keep coaching people like they are.
The best thing that can happen to a performer is you perform well when feeling like shit.
An athlete who runs a PR after feeling like trash. Or a parent who does their best writing with two kids under two.
It frees you up from the idea that you need to feel great to perform.

When is a first time seizure not the first time?
When you take a thorough history!
I saw someone recently who came in after their spouse heard a loud cry from sleep and some shaking of their limbs followed by confusion.
Pretty suspicious, right?
Even more so when you find out there's a history of stroke with some pretty noticeable encephalomalacia in the left temporal region.
So you go through your questions, asking about past episodes
You get a litany of No's, until, you get that answer you've been looking for
"Every night since I had the stroke 3 years ago I have been having panic attacks"
My ears perk up, I need to hear more about this
It turns out that every night for the past few years, about 1-2 hours after falling asleep this person has been waking up with an unpleasant feeling in their abdomen and living through unpleasant memories from their past.
This would last for a few minutes, and then he would go back to sleep.
This "first" seizure that brought them to the hospital wasn't the first. It was the first tonic-clonic seizure!
It's very common for people to have different kinds of auras (likely temporal in this case) which go undetected or get brushed off as nothing (by either the person experiencing them or the doctors they've seen).
Knowing about these past episodes can completely change your recommendations. So make sure to ask those probing questions, or as I usually begin, "I'm going to ask you some questions that may sound a little strange."

💊Early DOAC initiation is safe and non-inferior to delayed treatment regardless of infarct volume
❌⏱️No evidence that anticoagulation initiation should be delayed beyond 4 days on the basis of infarct size

Sight-threatening IIH: stent or shunt?
We still don’t know which is better. No randomised trials, variable data, and practice driven by local expertise.
An answer may be on the horizon.

Do you counsel patients on HLH when starting lamotrigine?
Rare, but life-threatening. Think the “3 Fs”: fever, falling counts, ferritin.
Two cases progressed to multiorgan failure within days of initiation.
