👉 High risk: CDK4/6-AI remains standard (more mature trial results). CAMBRIA 2 will define the role of CDK4/6-SERD combo
👉 Intermediate/high risk: SERD might be preferred (with more mature data) to CDK4/6. Is combo better? What about sequencing them (CAMBRIA1, EMBER4, ELEGANT)?
👉Intermediate risk: SERD preferred to CDK4/6

@ErikaHamilton9 presents primary results from HER2CLIMB-05: adding tucatinib to 1L maintenance HP significantly improved PFS; most benefit in ER-/HER2+ disease (delta 12 mo). More diarrhea and liver tox with tucatinib. New SoC maintenance option after induction for ER-/HER2+ MBC!

IHC lacks the dynamic range to predict outcomes with T-DXd, yet novel quantitative assays are emerging.
We presented data with HS-HER2 to predict T-DXd activity at #ESMO24. More data coming at #ASCO25.

There is an ongoing trial (STOPHER2) to look at stopping tx in people with HER2+ MBC who have been NED for a while, and there has been a study looking at changes in ctDNA with time in ER+ but I’m not aware of one looking at stopping tx if ctDNA undetectable . #SABCS24
