Is rCBF <30% still the definitive threshold for ischemic core in perfusion CT?
If the large core trials have taught us anything, perhaps it is that it's time to reconsider this "one-size-fits-all" standard.
This study by @KimBJstroke found that optimal rCBF thresholds for ischemic core estimation vary: rCBF <22% for immediate DWI and <30% for delayed DWI. The timing of recanalization affects threshold accuracy. Optimal thresholds may need to be tailored to the clinical setting.
The interval between CTP and recanalization was inversely correlated with the overestimation of ischemic core volume compared with the subsequent DWI.
