Hiyo ndg Kenny huioni kuwa ni shida kubwa.

3-Minyororo haiondoki kwa kuivunja kama huna nyenzo za kuvunja hasa ukiwa ni mtumwa uliyechoka na njaa na kiu; bali inafunguka kwa kutafuta “weak-link” yake ama kupata ufunguo wa kufuri. Na hivyo simaanishi kukubali unyonge bali utambuzi wa hali halisi.

3-Urusi siyo kwamba ilikuwa ni dhaifu!! Urusi ni taifa giant, kuanguka kwa Usoviet haiondolei Urusi uwezo wake katika anga za Uchumi na ushawishi wa kisiasa katika vitabu vya historia, Urusi ilikuwa ni Giant aliyeanguka na akaamka akapata balance na kujua kwamba hali imebadilika, hiyo ni tofauti na nchi kama Tanzania ama Kenye au mataifa mengi ya Afrika ambayo uhai wake wa Historia kama nchi haivuki karne, that’s History!

1-Kukubali kwamba huwezi kupata haki yako mbele ya mataifa yaliyokuzidi nguvu kwa kujisisimu misuli ye kujitegemea wakati katika uhalisia uwezo wa uchumi na hata rasilimali watu ikiwa duni, hiyo siyo “permanent inferiority” bali matumizi mazuri ya muda na akili.

2-Kama ombwe na siasa isivyoweza kujenga uwezo wa watu kujitegemea basi ukubali kwamba “Siasa ni uchumi na maisha” na hivyo owepo wa ombwe (political vacuum) katika nchi za Afrika ni uthibitisho wa udhaifu wa watu na maisha kiujmla, na kwamba huwezi kuwa na bargaining space wakati unayo njaa kali, huo ni uhalisia na siyo utukufu wa misimamo.

3-Urusi kabla ya kuanguka kwa Umoja wa Kisovieti, ilikuwa ni taifa lenye nguvu (uchumi na siasa) tofauti na Nchi za Afrika (mfn Wa nchi za Sahel), Urusi ilikuwa tayali na “bargaining space” toka katika historia yao tukufu na ilichofanya ni kukumbatia uhalisia kwa namna ya kwao huku wakiwa na uungwaji mkono na wananchi wao.

4- Ni ngumu leo kuisema Urusi moja kwa moja kwamba ni nchi ya kijamaa! Wanafanya biashara na nchi za magharibi, na wao pia wana operate kama Wavepari tu, Urusi hutunisha misuli kwa sababu misuri ya Urusi haikufa baada ya anguko la Usovieti, huo ni uhalisia na siyo hekaya.

5-Afrika na waafrika wenye mawazo ya kuinuka hapaswi kubeza ukweli wa historia ya mwanadamu kwamba “Utukufu ni kwa wale wenye nguvu” lakini nguvu haziji kwa kumbishia mwenye nguvu, bali kutafuta njia zenye uhalisia ili kupata nguvu hizo, dunia inawapa taji wote wanaojaribu na kujituma toka ndani yao na siyo kutegemea huruma au kukaidi uhalisia.

I’ll ignore the insults and respond to substance.

1. Geopolitics is not a morality play. It is not about fairness or equality. Nobody owes us equal treatment. It is about national interest. It is a negotiation arena through leverage, capability, credibility, and influence.

2. Yes, signals matter. Tweets, language, posture, headlines. But guess what? National interests are complicated. States execute their interests through layers and quite leverage. You cant draw conclusions from every tweet.

2. You talk about “changing power relations.” Fine. Power relations do change. But only through capability, not speeches. Nobody talked the US into power. Nobody talked China into strength. Nobody tweeted Russia into relevance. States earn bargaining power by building industry, knowledge, security capacity, and institutions that work.

3. Look at the Cold War. The Soviets did not build leverage by crying imperialism. They built it through science. That is how you fight for national interest. That is also why you miss what the US is doing in the Sahel. You are reading posture and ignoring structure. The US is not there for morality. It is there because interests moved. That is the whole point.

4. Yes, optionality only exists if you can say no. I agree. But a credible no is not a slogan. A credible no is a capability. If you cannot absorb the cost of saying no, then your no is just nothing. The way you earn the right to say no is to build the capacity that makes your yes valuable and your no costly to ignore.

5. What resistance are you talking about when your life runs on Western tech, Western finance rails, and Western security systems? The latest iPhone in your hand. This platform. The military and intelligence tools your government buys. That is not resistance. That is dependency with attitude. That is a mix of populism and hypocrisy.

6. You do not build a republic through the cheap us Vs them mentality. A republic is built by discipline. By institutions. By competence. By alliances that increase your capability. By refusing fights that burn your strength and choosing fights that expand it.

7. Look at Singapore. It did not become strong by shouting at superpowers. It became strong by making itself useful to all of them while building its own capacity. It welcomed Western capital. It also welcomed Eastern trade. It did not get owned by either side. It built world-class capacity. That is strategic optionality.

8. This is the difference you keep missing because you are so obsessed with us Vs them. If your politics is only posture, you will feel strong online and weak in reality. If your politics is capability, you will look quiet and become hard to ignore.

9. But first, you need to try be a very rational person, try to carry Tanzania’s national interests at heart, and then you might perhaps be able to have the clarity needed to see issues for what they really are. You will understand why Americans/Chinese/Europeans/Russian do what they do. They are not without fault but they go to the ends of the worlds to serve their countries. Can you?

In 2021, I was in church during a building project appeal. I had only 20k, but the way the pastor spoke about God’s favour stirred me. I gave 15k. That month was hard. No miracle money came, just silence and struggle.

Thank you for this intriguing case to think about.

The 25 year old patient, complaining of abdominal pain and feeling full after a meal looks very thin.

This is one image from an enhanced CT scan. The liver seems either compressed or the mass may be arising from it. Neither the spleen or kidneys or pancreas or vascular structures are visible.

Only a small amount of what looks like compressed colon is seen. The rest of the bowl is not identifiable. There is only slight elevation (arguably its within normal limits) of the left hemidiaphragm. The heart is midline.

There are no identifiable lymph nodes in this view.

Young people do get cancer. The most common cancers in young adults are: breast cancer, lymphomas (non-Hodgkin and Hodgkin), melanoma, sarcomas (cancers of connective tissues like muscles and bones), cancers of the female genital tract (cervix and ovary), thyroid cancer, testicular cancer, Colorectal cancer and brain and spinal cord tumors.
👉🏼 Neither primary or metastatic forms of these cancers usually present like this in a patient of this age. The possible exception would be ovarian cancer (which can displace intra-abdominal structures similar to a pregnancy.)

The apparent “mass” which fills most of the abdomen has the shape of the stomach to the level of about the pylorus. (It looks to be of the same density as the bladder, so it is likely cystic.) It pushes the colon down and towards the left.

Gastric outlet obstruction can result in a cyst like mass appearance in the abdomen due to gastric obstruction.

Gastric outlet obstruction may be due to mechanical causes e.g: due to a gastric bezoar from hair swallowing etc [though I would expect more air trapping and inhomogeneity, if that was the cause]; an obstructive gastric or duodenal polyp or a malignant tumour involving the gastric outlet; gastric volvulus [though in the one case I saw, the diaphragms were elevated much more in response to the distending stomach, until the stomach was untwisted]; pancreatic pathology such as a pancreatic pseudocyst or tumour; gastric outlet malignancy [she seems young for that]; or something weird like Bouveret Syndrome - which is gastric outlet obstruction secondary to impaction of a gallstone in the pylorus or proximal duodenum.

It could be a cystic or cyst-like lesion (such as a hematoma, lymphangioma or hemangioma) of the kidney, liver, pancreas, spleen or ovary. If it was ovarian I would expect the colon to be displaced upward, instead of squashed down. It could be a mesenteric cyst. They are rare but can be very big.

Besides mechanical obstruction and lesions of other organs, motility problems can cause an appearance of gastric obstruction.

Gastroparesis is the most common motility disorder causing gastric outlet obstruction. This is often associated with advanced diabetes mellitus but it can also be idiopathic. Other causes of gastroparesis include medication use (such as opioids, anticholinergics, and *glucagon-like peptide-1 agonists [which are used to treat obesity, but this patient looks very thin]), viral infections, paraneoplastic syndromes, connective tissue disorders, ischemia, certain inflammatory or neurologic conditions, and complications from surgery.

👉🏼 If I had to choose what I think is the most likely diagnosis (which I would never have done in real life due to the extremely limited radiological and clinical information) - I would be suspicious of either a tumour obstructing the gastric outlet arising from the gastric outlet area, or pancreas or biliary tract; or possibly a large right ovarian cyst.
