First ever locally injectable immunoactive agent with proven DFS benefit in localized prostate cancer.
Tandem randomized Phase II in surveillance forthcoming.

Presented at #ASCO26:
In high-risk localized prostate cancer, ADT plus apalutamide led to a pathological complete response or minimal residual disease and 5-year metastasis-free survival in a greater percentage of patients than ADT plus placebo. Full phase 3 PROTEUS trial results: https://t.co/UxXUkYwd8Z

🚨 PROTEUS: perioperative apalutamide moves into high-risk localized #prostatecancer
Just out in @NEJM
🧬 2109 men w/ high-risk localized or locally advanced PCa randomized to ADT + apalutamide vs ADT + placebo around radical prostatectomy.
🎯 Both stated primary endpoints met.
⚠️ But the trial deserves nuance. Thread 🧵

Cochrane updated its PSA screening meta-analysis. 789,086 men across 6 RCTs.
➡️ PCa-specific mortality: rate ratio 0.87 (moderate certainty)
➡️ Metastatic diagnosis: RR 0.65
➡️ Adverse events / QoL: little to no difference
Why this matters: 💡 PSA screening is back on stronger evidentiary footing for disease-specific mortality. Shared decision making still drives the recommendation downstream and additional tools (biomarkers and MRI) continue to improve the gap.

This was quite distressing. Two #RARP in a row for GG1 #prostatecancer...
Realise it's a technique session.
But surely we can't ignore poor decision-making?

"Good surgeon knows how to operate; excellent surgeon knows when (not) to operate"
But the crowd want it!

BRCA1/2 may be telling us a bigger story.
A new ESMO Open study suggests that pathogenic BRCA1/2 variants may increase the risk of less common cancers beyond breast, ovarian, pancreatic, and prostate cancer.
Signals observed:
BRCA1 → thyroid cancer
BRCA2 → bladder, head & neck, and skin cancers
The most striking finding:
BRCA2-associated bladder cancer risk appeared particularly high in females.
These data are hypothesis-generating, but they may reshape how we think about hereditary cancer predisposition and precision oncology.

New in our partnership with @DGlaucomflecken: the KEYNOTE-905 trial evaluated enfortumab vedotin plus pembrolizumab as perioperative therapy in patients with muscle-invasive bladder cancer who were ineligible to receive cisplatin. Read the full study for free: https://t.co/A1RikB7ZIo
📢 New publication in European Urology
We are pleased to highlight a new study published in European Urology, titled “IP8-FLUORESCE: A Prospective Paired Cohort Study Evaluating the Diagnostic Accuracy of Fluorescence Confocal Microscopy for Real-time Assessment of Surgical Margins in Radical Prostatectomy”.
👏 Congratulations to @NikhilMayor, @AJWLight, and Matt Winkler on this excellent contribution, together with all co-authors.

"Management of local prostate cancer (PCa) recurrence after primary radiotherapy (RT) remains challenging in daily practice. Salvage high-intensity focused ultrasound (S-HIFU) represents a valid option, but large prospective data are missing. "

⭐ Expanding theranostic targets in prostate cancer — beyond tumor cells
@OncoAlert@APCCC_Lugano#IPCS26#APCCC26@Silke_Gillessen@AOmlin
🔹 Next-generation targeting strategies are redefining the field
🔹 Key insights:
• GRPR emerges as a relevant target → especially in PSMA-low disease
• DLL3 opens a new avenue in NEPC-directed theranostics
• Clear movement toward phenotype-driven target selection
• Tumor microenvironment is now actionable → FAP as a stromal target
• FAP targeting may help overcome tumor heterogeneity and resistance
🔹 Take-home:
We are no longer targeting just the tumor —
we are targeting the ecosystem (tumor + phenotype + microenvironment)

🎧 Music during prostate biopsy = real clinical impact
🚨VIVALDI RCT in @EurUrolOpen
✅Patient-selected music during transperineal prostate biopsy lead to....
↓ Pain at every step (VAS consistently lower)
↓ Anxiety & distress post-procedure
↓ Sedation needs (37% vs 57%)
↑ Patient satisfaction & willingness to repeat
🫰Simple. Scalable. Zero downside.
⭐️So critical for anyone with a large Active Surveillance population
💡Nonpharmacologic interventions aren’t “soft”—they’re effective.
