ข้อมูลพันธุกรรมผู้ป่วยมะเร็งเต้านม-รังไข่ (HBOC) ชาวไทย ตีพิมพ์แล้วครับ เป็นข้อมูลขนาดใหญ่ชิ้นแรกของโครงการ Genomics Thailand รวมผู้ป่วย HBOC ชาวไทยได้แก่มะเร็งเต้านม, รังไข่, ตับอ่อน และต่อมลูกหมากกว่า 4,500 คน พบว่าผู้ป่วยที่มีประวัติชวนให้แพทย์สงสัยว่าเป็นมะเร็งถ่ายทอดในครอบครัว พบยีนกลายพันธุ์ได้บ่อย โดยเฉลี่ยผู้ป่วย 1 ใน 7 จะตรวจพบยีนกลายพันธุ์ และผู้ป่วยเพียง 2 ใน 3 เกิดจาก BRCA1/2 ที่เหลือ 1 ใน 3 เกิดจากยีนอื่น ที่สำคัญได้แก่ ATM, PALB2, TP53 โดยเฉพาะ ATM ที่พบบ่อยสุด แต่กลับพบน้อยในชาวตะวันตก และไม่อยู่ใน guideline ต่างประเทศ สนับสนุนว่าจำเป็นต้องมีการปรับให้เหมาะกับบ้านเรา

นอกจากนี้ยังพบผู้ป่วยมีการกลายพันธุ์มากกว่า 1 ยีนได้พอสมควร อาจสูงถึง 1:400-1:500 ซึ่งไม่เคยทราบมาก่อน

ผมว่านี่เป็นตัวอย่างที่แสดงให้เห็นว่าทำไมต้องมีข้อมูลจีโนมขนาดใหญ่ของเราเอง ถ้าไม่มี Genomics Thailand เราคงไม่รู้ว่าผู้ป่วยมะเร็งชาวไทยเจอยีนกลายพันธุ์ได้บ่อย, คงไม่รู้ว่ามียีนอย่าง ATM ที่พบบ่อยในคนไทยแต่ฝั่งตะวันตกไม่ค่อยให้ความสำคัญ ส่งผลต่อการวางแผน guideline และนโยบายการตรวจยีน นอกจากนี้ยังพบยีนกลายพันธุ์มากกว่า 1 ยีน ที่ส่งผลต่อการตรวจและคัดกรองสมาชิกครอบครัวของผู้ป่วยด้วย

หลังจากนี้คงจะได้เห็นข้อมูลของ Genomics Thailand ทยอยออกมากันเรื่อย ๆ และหวังว่าเราจะได้เห็นเฟสถัดไปของ Genomics Thailand ต่อไปในอนาคตครับ 🙂
