More immunotherapy disappointment in EGFR mutant NSCLC. Results from ORCHARD cohort of durvalumab + chemo after 1L osimertinib @JTOonline showed RR only 16%, PFS only 4.8m. Better options would be amivantamab + chemo, osi + chemo, dato-DXd / ADCs.

Interesting data supporting that the sclc transformation is much more aggressive than primary sclc. Why? Heavily treated patients? Heterogeneity? Definitely another Pandora’s box.

Disappointing early look at tarlatamab in #EGFR NSCLC after #SCLC transformation @JCOPO_ASCO from @alissajcooper et al. Small numbers but out of 14, only 1 response (7%). May reflect heavy pretreatment and PS but optimism somewhat tempered here.

Exciting news for our ROS1 patients — we now have a new, better-tolerated option: taletrectinib! The next big challenge will be overcoming resistance.

FDA approves taletrectinib for #ROS1 NSCLC based on TRUST I and TRUST II trials showing 1L RR 85-90% with 63-72% of responses lasting ≥12 months and post TKI, RR 52-62%. Taletrectinib has selectivity over TrkB so should have less neurotoxicity.

Dr. @ClaudiaProto83 provides a great overview on thymic cancers at #ITCD2024. Surgery preferred. RELEVENT trial showed impressive outcomes with 1L carboplatin + paclitaxel + ramucirumab. Some targeted agents with activity. Immunotherapy with modest activity but toxic.
